Depends on which civilization you are referring to since there were (as now) many forms of government. It is believed the idea of government was an extension of the family unit (elders, father, mother, kids in descending order). This led to a ‘clan’ type of unification, which further developed into areas/regions. If someone didn’t like it, they could leave and find another group to join.
From these beginnings came monarchies, oligarchies, democracy, republics, and many other forms.

Hierarchy; because most of it is to do with power/royality/god.
